Position Summary:
LVN – North Runnels Home Health
As a Licensed Vocational Nurse for North Runnels Home Health, you are responsible for performing skilled nursing visits, documentation, and community projects. Working independently, you must be knowledgeable of current nursing and medical developments, changes in health care legislation, regulations, and administrative policies. You will comply with changes to North Runnels Home Health policies and procedures. The Licensed Vocational Nurse is a torchbearer for company values, and will represent North Runnels Home Health in the community.
I. Essential Job Functions:
Must be available to agency personnel at all times in person or by telephone during business hours
Performs skilled nursing services regarding client care, scheduling, documentation, community projects.
Submits required paperwork by the deadline to facilitate operations of the agency
Participates in the on-call program
Follows a client’s plan of care as written
Assures that a reassessment of a client’s needs is performed when there is a significant health status change in the client’s condition, at the physician’s request, or after hospitalization, if required
Recognizes and follows the appropriate communication and organizational chain of command of the company
Communicates with referral sources to facilitate client services
II. Required Skills, education, Certification, License, and/or Experience
Must maintain an active (unrestricted) LVN license in the working state(s) with at least one (1) year of verified experience in medical surgical or another nursing discipline
A genuine interest and ability to work with people of all socio-economic groups
Maintain a valid driver’s license and own a legally registered vehicle that holds the minimum liability insurance coverage
Strong skills in problem solving and interpersonal communication
Excellent supervisory skills, time management skills, and detail oriented
Able to read, write, and comprehend English
III. Working Conditions and Physical Requirements
Must be able to work in a work environment that involves intermittent body movements such as walking, standing, sitting, driving, bending, lifting up to 50 pounds, stooping, kneeling, climbing stairs, and reaching. Must have dexterity and strength in hands/limbs for reasonable accommodation to perform required tasks. The Nature of duties presents moderate exposure to infectious and chemical hazards or job-related accidental injury. Must be able to travel to perform field visits or travel between offices if necessary. Not all sites/locations will be handicapped accessible. Driving is a requirement.
IV. Compliance
The Licensed Vocational Nurse is required to fully comply with the state and federal regulations, contract standards, and North Runnels Hospital District and North Runnels Home Health agency policies and procedures. All nursing personnel is responsible for being familiar with the contents of the Nurse Practice Act and Board Rules.
